GRANT SUPPORT
VA HSR&D Locally Initiated Project award (SEP-DEC 2002, Laurel A Copeland, PI). $10,000. "Changes in Stress-Related Diagnoses and Health Services Use in Response to 9/11." Principal Investigator. Outpatients were randomly sampled and surveyed to assess veterans' perceptions of their health care in VA clinics in New York City and Ann Arbor, Michigan in the year following September 11, 2001. The study was designed to assess the effect of close proximity to the 9/11 disaster on perceptions of health, health services access, and satisfaction with care, among veterans who sought care for symptoms related to 9/11 and those who did not. Demographic characteristics and psychiatric symptoms of PTSD were measured as covariates.

VA TRX-01-091 (2003-2008, Valerie A Lawrence, PI). $735,400. "Systematic Review and Tracking Database for CPG Implementation Research." Co-investigator. Systematically review the literature on organizational strategies of implementing evidence-based medicine into clinical practice; designed to improve the quality of care provided to veterans with QUERI diseases - high prevalence, chronic conditions (e.g., diabetes, CHF, colon cancer, schizophrenia, depression).

VISN Implementation Project XVA-66-011 (2005-2008, Stephen L Holliday, PI). $586,442. "Implementing Integrated Behavioral Health Care for Predominantly Hispanic OEF/OIF Veterans." Co-Investigator. Provide comprehensive, integrated care for returning OEF/OIF veterans, including screening and treatment for substance abuse (alcohol, tobacco, other drugs), depression, PTSD, and medical care as well as job services. The target population lives in an underserved region of South Texas near the Texas-Mexico border. Outreach efforts and sensitivity to cultural issues will characterize the 3-year evaluated implementation project.

VISN Implementation Project XVA-66-012 (2005-2008, Stephen L Holliday, PI). $736,990. "Improving Substance Abuse Treatment in the Borderland: Early Intervention and Follow-up in Remote Primary Care Clinics." Co-Investigator. Provide substance abuse interventions in a stepped care program for veterans in remote clinics near the Mexico-Texas border. This demonstration project will seek out at-risk veterans to engage them in treatment for substance abuse (alcohol, tobacco, other drugs). The 3-year evaluated implementation project will augment and integrate with clinical care currently being offered in three VA satellite clinics in an impoverished, largely Hispanic population.

VA HSR&D Career Development Award MRP-05-145-2 (JAN 2006-MAR 2009, Laurel A Copeland). $300,000. "Patterns of Late-Life Healthcare among VA Patients with Schizophrenia." Study current patterns of inpatient and outpatient medical, psychiatric, and other care for VA patients over age 50 with schizophrenia with or without diabetes, and the relationship of care patterns to mortality.

VA HSR&D Project IIR-05-121 (APR 2006-JUN 2009, Polly H Noel, PI). $518,100. "Obesity Care Practices in the Veteran's Health Administration." Co-Investigator. Describe variations in the provision of obesity care practices in the VHA and examine system, provider, and patient-level factors that predict these variations; examine the impact of obesity care practices on BMI and other important clinical outcomes; and identify longitudinal patterns (latent classes) of BMI trends over time and predictors of latent class membership.

VA HSR&D Project IIR-05-326 (JUL 2006-MAR 2010, Laurel A Copeland, PI). $314,200. "Patterns of Late-Life Healthcare among VA Patients with Schizophrenia." Analyze administrative data (FY02-FY05) on veterans age 50 or older receiving VA care for schizophrenia, diabetes, or both schizophrenia and diabetes to compare patterns of care (inpatient, outpatient, medical, psychiatric), compare prescription profiles (e.g., use of antipsychotic or antidiabetic agents) and clinical indicators (e.g., blood glucose, A1c testing, lipids), and assess the impact of patterns of care and intermediate measures on mortality among the three groups of patients.

VA HSR&D Project IIR-06-062 (2007-2010, Mary Jo V Pugh, PI). $704,800. "Inappropriate Drug Use for Seniors: Should VA adopt new HEDIS measures?" Recently, the National Committee on Quality Assurance developed measures to identify potentially inappropriate prescribing in the elderly (PIPE). These measures (2006, 2007 HEDIS PIPE) are controversial and not strongly linked to patient outcomes. Because of methodological problems with prior studies this study will assess the link between HEDIS PIPE exposure and patient outcomes in cohorts of older veterans with incident exposure to drugs included in these measures. Role: Co-Investigator (10% effort)

NINR R01 1R01NR010828-01A1 (2007-2010, Eric M Mortensen, PI). $323,538. "Impact of Statins and ACE Inhibitors on Outcomes for Pneumonia and Sepsis." Community-acquired pneumonia (CAP) and sepsis are leading causes of death in the US. This study will determine the impact of statins, ACE inhibitors, and angiotensin receptor blockers (ARBs) on CAP- and sepsis-related mortality. Role: Co-investigator, Data Core Lead (10% effort)

Department of Defense (2008-2013, Alan Peterson, PI). $25,000,000. "STRONG STAR (South Texas Research Organizational Network Guiding Studies on Trauma And Resilience)" Co-investigator. STRONG STAR is a multidisciplinary and multi-institutional research consortium dedicated to the development and evaluation of prevention and treatment approaches for combat-related PTSD in active-duty military and recently discharged veterans.

"Tracking OEF/OIF Transition from DOD to VA" Establish feasibility of PHI transfer between DOD and VA medical treatment facilities on a local level; track transition of seriously wounded warriors from BAMC into VA; determine healthcare use in VA by OEF/OIF veterans focusing on mental health care.

Hogg Foundation for Mental Health Research Grant for Junior Faculty #HE-021-2009 (JUN 2009-MAY 2010, Laurel A Copeland, PI). $14,995. "Mental Health Treatment Preferences of OEF/OIF Veterans in South Texas." Characterize mental health care associated with optimal outcomes for post-deployment veterans returning from Iraq or Afghanistan and receiving care in the South Texas Veterans Health Care System.

DOD CDMRP #09090014 (SEP 2009-MAR 2010, Toby Cooper, PI). $282,040. "Drug Related Overdoses Among a Military Population".The study will analyze epidemiological data on prescription medication abuse for a cohort of active-duty OEF/OIF and non-OEF/OIF soldiers, examining comorbid PTSD and TBI from combat exposure to identify risk factors of self-poisoning in the population of military personnel.

VA HSR&D Project HX-09-335 (APR 2010-MAR 2013, Laurel A Copeland, PI). $792,500. "Surgical Treatment Outcomes for Patients with Psychiatric Disorders (STOPP)." Analyze surgery experiences of VA patients with and without severe mental illness (i.e., schizophrenia, bipolar disorder, major depressive disorder, post-traumatic stress disorder).

VA HSR&D DHI-09-237 (10/2010-09/2014, M J Pugh - PI). $1,099,071. "Identifying and Validating Complex Comorbidity Clusters in OEF-OIF Veterans." TRACC will study trajectories of physical and psychiatric comorbidity clusters for our new veterans receiving care in the VHA, identifying factors in stable vs deteriorating trajectories and their association with resiliency, coping and social support.

Scott & White Research Mentor Award (01/01/2012-12/31/2014, L A Copeland - PI). "Homefront: Implementation and Evaluation of a Post-deployment Family Health Program." $10,000. This study will: (1) describe the development and implementation of Homefront services ; and (2) assess the association of Homefront counseling services with change in symptoms.

Commonwealth Fund (08/01/2012-12/31/2014, R Reid - PI). "Hybrid Strategies to Improve Health Care's Triple Aims: Lessons Learned From the Transformations of Two Health Systems." $347,742. The purpose of this study is to identify trends in how youth are screened and treated for lipid disorders during 2001-2012. VA HSRD HX-12-358 (01/2015-12/2017, M Hawn - PI). $900,000. "Improving Surgical Quality: Risks and Impact of Readmission." This study will (1) determine the incidence, costs and variations in readmission following inpatient surgery, (2) estimate contributions of comorbidity, procedures, complications and facility characteristics to readmission, and (3) develop and validate a risk stratification tool. Scott & White Research Mentor Award (03/2015-03/2017, L A Copeland - PI). $2948. "Hypertriglyceridemia: Relationship with Acute Pancreatitis and Cardiovascular Disease." This study will determine the risk of acute pancreatitis (AP) and cardiovascular disease (CVD) as a function of triglyceride level and to characterize the impact of lipid-lowering therapies (LLT) prescribed as modulators of this risk.

HMORN-OAIC AGING Initiative Pilot Project Grant, National Institute on Aging Grant 1R24AG045050-01A1 (04/2015-03/2016; L A Copeland - PI, C Quinn - OAIC Partner). "Diabetes, Dementia, and Multiple Chronic Conditions in Males with Hip Fracture." $39,104. As part of an ongoing agenda to reduce readmission rates among older adults seeking care in integrated payer-provider systems, we will use large database analysis to determine the association of comorbid diabetes, hyperglycemia, dementia, and multiple chronic conditions with outcomes of hip fracture surgery (30-day readmission and mortality), adjusting for demographic and clinical covariates including cognitive-impairment-non-dementia (CIND), cardiovascular disease, and use of physical therapy.
